Please follow the instructions below to help you fill out the form successfully:
1. Log on to: http://my.aucegypt.edu.
2. Use your AUC email username and password to access your account.
3. On first login, a pop-up window will appear asking you whether you want to add content to your homepage or not. Click “Add Content.”
4. From the menu on the left of the webpage, choose “AUC Services.”
5. Select “Online Forms.”
6. Select the correct form.
7. Fill in the form and click continue to print as a PDF.
8. Save the form on your computer.
9. Attach the form to an email, and send it to the person in charge (you will find the name and the email of the person in charge at the top part of the form).
